Understanding thickened and dystrophic nails and your options.
A dystrophic nail is one that has changed in shape, thickness, or texture. It may look ridged, crumbly, curved, or very thick. Fungus is one cause, but it is not the only one.
People often notice these nails after an injury or as they get older. The nails can be hard to trim and may press on the skin inside shoes. A proper check helps us find the cause and choose the right care.

Diagnosis and care
How we treat thickened and dystrophic nails.
Step 1 · Diagnosis
We look at every affected nail and ask about injuries, health conditions, and shoes. We may take a small nail sample to test for fungus. If we suspect a bone or circulation issue, imaging or extra testing can help.
Step 2 · Treatment
Regular gentle trimming and thinning of the nail can relieve pressure and pain. If fungus is present, we treat it as well. Padding, roomy shoes, and toe protection can reduce rubbing. Good nail care at home, done softly, also helps.
Surgery is not often needed. If a nail stays painful or keeps causing problems, a minor procedure to remove part or all of the nail may be discussed.
Recovery
What to expect over time.
Nail changes can take many months to grow out, and some nails never look fully normal. Comfort usually improves sooner than looks.
When to book
Do not wait it out.
See a podiatrist if a nail changes shape or color, or if it hurts. Come in sooner if you have diabetes or notice redness, swelling, or an open sore near the nail.

Your questions answered.
Is a thick nail always fungus?
No. Injury, skin conditions, and age can also cause thick nails. Testing shows which cause is behind it.
Will my nail ever look normal again?
It may improve, especially if the cause is treated. Long term damage can leave some changes behind.
